Evaluation form is asked by many institutes to be submitted along with the elective application. Evaluation form means the form on which, at the end of your elective, the host medical school will evaluate your performance and usually they send it back directly at your home school.
Sometimes the host medical school gives its own evaluation form and the problem gets solved. But sometimes you have to give one of your home medical institute.
Unfortunately, most of the medical schools in Pakistan, India etc do not have an evaluation form of their own, because there was no tradition of going for electives few years ago. Therefore, to solve that problem here is a sample evaluation form that can be used if required for your application. Download the file below!

If its to show to your medical school only then whats the use of it because in Pakistan it is worthless as no school requires you to or give credit for doing electives in USA. Please let me know if I'm wrong.

it is the same for everyone... keep in mind that evaluation form is basically a document from the home medical school where you study.... not the university/ medical school where you are visiting for electives.... tc n best of luck!
